How do I provide volunteers with proof of their work and volunteer hours?

I work for a very small nonprofit and we just started our volunteer hours. All of our volunteers work and hours are accounted for and kept safe. I would love a template outlining what to include in a document meant to be used as proof of their volunteer hours. Can anyone help me with this?

Habitat for Humanity has just the form. Log on to any habitat site and you should find the form. The form provides the name and address and email and the hours worked. Its also a permanent copy for the non-profit business so they can contact them again in the future.

    Are you talking about keeping track of their community service ordered by a Judge? I would just write out a slip at the end of each week telling them how many hours they were credited for.
